DULCE Case Manager

I’ve been with SAPCC since the fall of 2021!

I’m the DULCE Case Manager, embedded in the Mt. Ascutney Pediatric Clinic, supporting families with babies 1–12 months old. 👶 Right now, we’re supporting 66 families—and that’s just the little ones! I connect with families during well-child visits, home visits, follow-up calls, and I help with appointments, weight checks, and guidance around things like starting solids. 🍼🥄

I also support families with postpartum care, mental health, food, housing, transportation, finances, and more, connecting them to resources like WIC, legal aid, and The Women’s Resource Center. 💪✨ Plus, I distribute essential items throughout the first year to help families thrive!

What I love most? ❤️ Being part of the DULCE program lets me combine case management with the medical side I love. Seeing families embrace the program and trusting me during some of the most vulnerable moments of their lives is incredibly rewarding
